Forms to support students, applicants, and counselors throughout the admissions process.
Fee Waiver Request
The Fee Waiver Request allows high school counselors, college counselors, and other school officials to request an application fee waiver for a student based on demonstrated financial need.
- May only be submitted by a school official
- Available through the counselor portal
- International students are not eligible for fee waivers
School Explanation Form
High school counselors may use the School Explanation Form to provide context about school-wide changes or circumstances that are not already addressed in the school’s profile.
- Available through the counselor portal
- Intended for school-level information, not individual student circumstances
Student Explanation Form
The Student Explanation Form allows counselors to verify or explain extenuating circumstances that may have affected a student’s academic record.
- Available through the counselor portal
- Intended for documented student circumstances
- Should not be used as a general recommendation letter
Admission Appeal Form
Students may submit an admission appeal if they were denied admission or were not admitted to their first-choice major.
- Available through myIllini
- Intended only for students with new and compelling information not included in the original application
- Appeals are reviewed for the current application cycle only
Biographical Change Form
The Biographical Change Form allows applicants to update or correct personal information submitted with their application.
- Available through myIllini
- Requires an active application for the current admission cycle
Program/Term Change Form
Applicants may request consideration for a different academic program or entry term through the Program or Term Change Form.
- Available through myIllini
- Requires an active application or admission offer for the current admission cycle
- Program change requests require updated responses to any major-specific application questions
Course Change Form
The Course Change Form allows applicants to report updates to their senior-year or college coursework after submitting an application.
Students may use this form to:
- Report schedule changes
- Add or remove courses
- Correct self-reported courses and grades (first-year applicants)
If a decision has already been made and the changes could affect that decision, an admissions staff member will contact the student. If the application is still under review, the updated information will be considered as part of the admission process.
Students who have already been admitted should not change their senior-year schedule without prior approval from our office.

Delayed Admission Form
The Delayed Admission Form allows newly admitted, degree-seeking students to request a postponement of their enrollment.
- Available through myIllini
- Delays of one or two semesters may be approved
- Students with extended U.S. military commitments may be eligible for delays of up to two years
Requests are reviewed individually and may be approved for circumstances such as:
- Medical emergencies
- Serious health conditions
- U.S. military orders
Requests generally will not be approved for:
- Gap years
- Attendance at another academic institution
- Financial challenges
- Visa issues
- Travel or flight-booking complications
- First-year students who will not graduate before classes begin at Illinois
Students should submit a request as soon as they know a delay is needed and no later than one month before the start of their admitted term.

Application Withdrawal Form
Students who no longer wish to be considered for admission may withdraw their application before a final admission decision is released.

- Available only to applicants with an active application
- Does not allow an application to be withdrawn and replaced with a new application for the same admission cycle
